Profiling a Waste Stream

To begin the waste stream approval process please complete, sign, and date a TMDP profile form and a UTS form (required for all streams carrying D-listed codes).  Forward the completed forms to your account manager or customer service representative. Once our approvals group reviews your profile deliver a minimum one-quart representative sample (required for all bulk streams) to:
